General Medicine Internship Real Patient OSCEs Towards Optimizing Clinical Complexity


This is online E log book to discuss our patient's de-identified health data shared after taking his/her/guardian's signed informed consent. Here we discuss our individual patient's problems through series of inputs from available global online community of experts with an aim to solve those patients clinical problems with collective current best evidence based inputs .This e-log book also reflects my patient centered online learning portfolio and your valuable inputs on comment box is welcome.









Introduction: This online E-log Entry Blog is an objectively structured clinical examination method to assess the clinical competence during the course of my General Medicine Internship rotation (June'2023-August'2023) by reviewing the case reports shared below and to discuss, understand and review the clinical scenarios and data analysis of patients so as to develop my clinical competency in comprehending clinical cases, and providing evidence-based inputs for questions surrounding the clinical vignettes borrowed from the E Log Book.



Note: The cases have been shared after taking consent from the patient/guardian. All names and other identifiers have been removed to secure and respect the privacy of the patient and the family.






CaseReport-1:
30 year old male patient resident of suryapet and a watchmen by occupation presented with chief complaint of Loose stools and vomitings since yesterday morning and shortness of breath since yesterday evening.

1)What is DIABETIC KETOACIDOSIS?


Diabetic ketoacidosis is a serious complication of diabetes.The condition develops when the body can't produce enough insulin. Insulin plays a key role in helping sugar — a major source of energy for muscles and other tissues — enter cells in the body.Without enough insulin, the body begins to break down fat as fuel. This causes a buildup of acids in the bloodstream called ketones. If it's left untreated, the buildup can lead to diabetic ketoacidosis.

3)WHAT ARE THE CAUSES OF DIABETIC KETOACIDOSIS?


Diabetic ketoacidosis usually happens after:

  • An illness:Pneumonia and urinary tract infections are common illnesses that can lead to diabetic ketoacidosis.
  • A problem with insulin therapy
  • Physical or emotional trauma
  • Heart attack or stroke
  • Pancreatitis
  • Pregnancy
  • Alcohol or drug misuse, particularly cocaine
  • Certain medicines, such as corticosteroids and some diuretics
